Who is authorized to sign the self-certification statements?

The document must be signed by the CEO, President, CFO, or equivalent officer of the applying entity. Not all listed officers are required to sign. Signature from any one of these roles is sufficient. However, if financial statements were provided by a qualified parent entity, then they must be co-signed by the parent entity’s CEO, President, CFO, or equivalent officer.

It is up to the applicant to identify the signatory as an equivalent officer within their operating model and local jurisdiction. If during the evaluation there are questions on whether the document meets the signature requirements, then clarifying questions may be issued, and the applicant may be asked to submit documentation to confirm that the person holds an equivalent position.
