In the case that an applicant does not make an attestation as part of the self-certification document, the applicant must provide an explanation as to why they did not make the attestation and why the self-certification document should still be considered valid and appropriate. An appendix statement and supporting documentation (if applicable) included in the attachment would be acceptable.
